What Is It?

The Functional Process Note Assistant is a practice tool designed to support clinicians using Acceptance and Commitment Therapy and process-based approaches.

 

It provides structured guidance for writing clinical notes, reflecting on sessions, and planning next steps while staying oriented to function rather than form.

This tool was developed to support clarity, consistency, and clinical integrity across varied cases and settings.

Who Is It For?

This tool is designed for:

  • Clinicians using ACT or process-based approaches 

  • Supervisees and trainees learning functional conceptualization

  • Experienced practitioners seeking greater clarity and consistency in documentation

 

It is not designed to replace clinical judgment, supervision, or individualized case formulation. It is a support, not a script.

That boundary matters.

Why It Exists

Traditional documentation often pulls clinicians away from function and context, reinforcing symptom lists, diagnoses, and static formulations. Over time, this can erode clinical clarity and disconnect notes from the actual work happening in the room.

The FPN Assistant exists to counter that drift by supporting documentation that reflects:

  • Context over content

  • Function over form

  • Process over protocol​

​

It was primarily developed for the RIACT Residency Program both to better tract a session as well as to get immediate feedback.

How It Is Used

The FPN Assistant guides clinicians through:

  • Functional analysis of key patterns

  • Process-based intervention planning

  • Trajectory tracking over time

It can be used:

  • As a learning tool for trainees

  • As a consistency check across cases
